Online Health Screening Tool

For our families who’ve chosen hybrid learning: Please add the DOE’s Online Health Screening Tool to your smartphone. Use the link to open the screener, then click “Add this tool to your phone.” The DOE is mandating that students and staff either show the results of the screening from their phone or show us a completed paper form before entering the school building. For those without an email or paper form, a temperature check will need to be taken by school staff before a child can enter the school building.

To sign in and begin the 4-question survey on the Health Screening Tool, use your child’s DOE email username (the part before the @) and password. If you have more than one child, you can open each child’s survey on a different browser or take screenshots to save time. We recommend that this be done in the morning before leaving the house (the survey must be submitted on the same date your child is attending school).

In order to keep everyone healthy and slow the spread of COVID-19, students will not be able to attend in-person classes if they have:

  • Experienced any symptoms of COVID-19, including a fever of 100.0 degrees F or greater, a new cough, new loss of taste or smell, or shortness of breath within the past 10 days;
  • Received a positive result from a COVID-19 test that tested saliva or used a nose or throat swab(not a blood test) in the past 10 days;
  • Been in close contact (within 6 feet for at least 10 minutes) with anyone who tested positive for COVID-19 or who has or had symptoms of COVID-19 in the past 14 days; or
  • Traveled internationally or from a state with widespread community transmission of COVID-19 per the New York State Travel Advisory in the past 14 days.

As always, please keep your child home if they are sick.
