For school year 2021-22, all students must show a completed health screener before they can enter the building. The easiest way to do this is to add the DOE’s Online Health Screening Tool to your smartphone and complete the short survey each morning. Use the link to open the screener, then click “Add this tool to your phone.” The DOE is mandating that students and staff either show the results of the screening from their phone or show us a completed paper form before entering the school building.

To sign in: use your child’s DOE email username (the part before @nycstudents.net) and password. Or sign in as a guest. Kindergarten students, new students or those who didn’t previously activate their DOE email will have to log in as guests for the first few weeks of school until they get their IDs and DOE email accounts. Do not try logging in with your NYCSA/mystudent.nyc ID or your child’s ps321students email. It won’t work.
If you have more than one child, you can open each child’s survey on a different browser or take screenshots to save time. We recommend that this be done in the morning before leaving the house (the survey must be submitted on the same date your child is attending school).
In order to keep everyone healthy and slow the spread of COVID-19, students will not be able to attend in-person classes if they have:
- Experienced any symptoms of COVID-19, including a fever of 100.0 degrees F or greater, a new cough, new loss of taste or smell, or shortness of breath within the past 5 days;
- Received a positive result from a COVID-19 test that tested saliva or used a nose or throat swab(not a blood test) in the past 5 days;
- Been in close contact (within 6 feet for at least 10 minutes) with anyone who tested positive for COVID-19 or who has or had symptoms of COVID-19 in the past 5 days.
