It is hard to believe that our 5th graders will soon wrap up their time at PS321.

• Confirm your child’s yearbook, diploma, and t-shirt name
• Select your child’s t-shirt size
• Submit your 5th Grade Dues
  

We have some exciting activities planned for our graduating students this spring. Your child will also receive a yearbook, class T-shirt & hat and graduation certificate.

The 5th Grade Activities fee is $100. Please consider adding to your dues payment to support our scholarship fund that ensures every student participates.  

For parents with multiple children in 5th grade – please fill out one form per child.
