Matching Contribution Form

If you have requested a corporate match for any of your donations, please provide the following information to help the PTA Treasurers process and track your match.

Every company has their own way of processing these requests, often through outside organizations, so it’s helpful to track your match through your company and then follow up with the treasurers if needed.

  • Please let us know the amount of your donation. If you gave to more than one event in a calendar year you can submit all here. Please list the amounts separately so we can verify them.
  • Many companies send matching gifts through Benevity or American Express. Ask your HR department if the check will come from the company you work for or another bank or organization. Any information you can provide will help us track your match.
