Spring Carnival Sponsor Payment Form

Thank you so much for your commitment! We appreciate your participation in the PS321 PTA Spring Carnival, which will occur on Saturday, June 1 from 10am-4pm. Your generous support will not only help us bring this beloved celebration to our community, but will also help us to keep class sizes small and fund art, music, chess and after-school programs at PS321 throughout the school year.

The PS 321 PTA is a federally recognized not-for-profit organization under section 501(c)3. All donations are tax deductible to the extent the law allows.

Please complete this form to secure your participation via credit card (incl. credit card fee) no later than Friday, May 17. Or if you prefer, you may make payment by mailing your check to: PS321 PTA, c/o Treasurer, 180 7th Avenue, Brooklyn, NY 11215 or delivering your check in person via a mutually convenient time and location for a PTA representative to connect with you. Sponsorships are non-refundable.
